Housing Cancellation Application

DO YOU PLAN TO LIVE OFF-CAMPUS FOR SPRING 2009? OR ARE YOU WITHDRAWING FROM THE COLLEGE?

If you  plan to be an off-campus commuter, or if you are Withdrawing from the College or taking a Leave of Absence, you must complete a Housing Cancellation Application and submit it to the Office of Residence Life.

SPRING SEMESTER: Students who withdraw from campus housing for the Spring semester may do so with no penalty by notifying the Office of Residence Life before December 1st. On or after December 1st but before the first day of spring classes, students who withdraw from Spring semester housing will be charged $150. Students changing residency status on or after the first day of spring classes will be charged $150; also, charges/refunds for room and board will be prorated.

After the change in residency status from “resident” to “non-resident (commuter)” has been approved, the Office of Residence Life will notify all other administrative offices (Business Office, Aramark, Security & Public Safety, Office of Student Affairs). Any room and board charges/refunds will be determined by the policies in the Lake Forest College Bulletin. Any other charges/fines upon checking-out of the residence halls will be determined by policies in the Student Handbook.

You must keep the Office of Student Affairs advised of your current off-campus/local address and phone number at all times.

ARE YOU PARTICIPATING IN AN OFF-CAMPUS STUDY PROGRAM?

APPROVED OFF-CAMPUS PROGRAM FOR SPRING 2009: If you have been approved and intend to participate in an off-campus study abroad program for Spring 2009, you must complete a Housing Cancellation Application for the Spring 2009 semester in order to avoid penalty charges. Also, since you will not be on-campus to select your room for the 2009-10 academic year during the Room Selection period in April 2009, please complete the proxy information on the Housing Cancellation Application and submit an On-Campus Housing Application.
